Information you choose to share

Please do not submit Social Security numbers, payment details, medical history, or policy documents through this form.

How your inquiry is handled

Your submission is sent through Formspree so Kathy Lee / KJLEE Insurance Group can review your inquiry and contact you about it. This is a request for a conversation and does not create an insurance policy or coverage.

Formspree processes form submissions under its own privacy policy. Website hosting and font providers may process technical information needed to deliver this website. This page does not use advertising trackers or save your form answers in browser storage.
